Eligibility
If you are a regular full-time employee of Lochmueller Group, you are eligible to participate in the benefits program on your 30th day of employment. As a new hire, you have the option to enroll yourself and/or your eligible dependents in Lochmueller's benefits program. If you do not enroll within 30 days of your initial eligibility period, you will not have an opportunity to do so until the next open enrollment period.

Coverage will end at midnight the day of termination or resignation.

If you don’t enroll by the date when your benefits will go into effect, you will not have the opportunity to do so until the next open enrollment period, unless you have a Qualifying Life Event.​

Eligible dependents include:

Your spouse (Spouses who are eligible for coverage under their Employer’s Health Plan are NOT ELIGIBLE to enroll in Lochmueller Group Health Plan for PRIMARY COVERAGE. Secondary coverage is permitted.)

Your children up to age 26. Only those dependents who meet the eligibility requirements can enroll for coverage.
